Monsplasty Sydney

Specialist plastic surgery in Sydney.

Monsplasty

Monsplasty is surgery to remove excess skin and fat from the mons pubis, the soft area over the pubic bone, and to reposition the remaining tissue so it sits flatter and higher. Dr Maryam Seyedabadi, Specialist Plastic Surgeon, performs monsplasty under general anaesthetic at accredited hospitals in Sydney.

The mons commonly changes after pregnancy or a significant change in weight. Skin stretches, fat deposits shift, and the tissue can hang forward or downward, sometimes overhanging the pubic area. That can cause chafing and rubbing, trap moisture, make hygiene harder, and be uncomfortable during exercise or cycling. It can also be visible under clothing, swimwear and activewear, and appearance is a legitimate reason to ask about the procedure.

Monsplasty is often combined with abdominoplasty, because lower abdominal surgery can pull the mons downwards if it is not addressed at the same time. After examining you, Dr Maryam may advise that your anatomy is within a normal range and that surgery is not the right option, or that stable weight or a different procedure should come first.

Who it may suit

  • Excess skin and fat over the mons pubis after pregnancy, or after weight loss through dieting or bariatric surgery
  • A mons that hangs forward and is noticeable under clothing, swimwear or activewear
  • Chafing, rubbing, moisture, skin irritation or recurrent rash in the crease beneath the mons
  • Difficulty with hygiene, or with exercise and cycling, because of tissue bulk
  • Patients already planning abdominoplasty who want the mons addressed in the same operation
  • A mons that appears pulled down or heavy after previous abdominal surgery

What monsplasty involves

Monsplasty is performed under general anaesthetic. On its own it often takes one to two hours and may be day surgery. Combined with abdominoplasty it takes longer and usually involves an overnight stay or more.

  1. Dr Maryam marks the incision with you standing, so the scar can be placed low and covered by underwear, often along or within an existing abdominoplasty or caesarean scar where one exists.
  2. Excess fat is reduced, by direct excision or liposuction or both, depending on tissue quality and volume.
  3. Excess skin is removed and the remaining tissue is lifted and secured to the deeper layers so the mons sits higher and flatter.
  4. The position of the pubic hair-bearing skin is checked during surgery so it is not drawn too high or left too low.
  5. The wound is closed in layers. A drain and a compression garment may be used, particularly with combined surgery.

Recovery

Recovery depends on how much tissue was removed and whether other procedures were done at the same time. The points below are a general guide rather than a fixed timeline, and combined surgery usually takes noticeably longer.

  • Swelling, bruising and oedema are usual for roughly the first two weeks, with firmness and residual swelling settling over several months.
  • Many patients take around two weeks away from office work when monsplasty is done alone, and often longer when it is combined with abdominoplasty or their work is physical.
  • A compression garment is generally worn for some weeks, and short walks are encouraged early to reduce clot risk.
  • Lifting, core work and strenuous exercise are commonly restricted for about six weeks, on Dr Maryam’s advice.
  • Numbness across the mons and lower abdomen is common early and usually improves gradually, although some areas may remain altered.

Risks and considerations

Risks include bleeding, haematoma, seroma or fluid collection, infection, delayed healing or separation at the scar, wide or thickened scarring, and reduced or altered sensation in the mons and lower abdomen. Contour irregularity, residual fullness, asymmetry, or a scar or hairline that sits higher than planned can occur, and revision surgery is sometimes needed. Fat necrosis is uncommon but possible.

General anaesthetic and longer operations bring added risk, including deep vein thrombosis and pulmonary embolism, and that risk increases with combined surgery, smoking, higher body weight and some medical conditions. Dr Maryam will explain the preventive measures used, your individual risk, and what monsplasty will and will not change. No result can be promised.

Before your consultation

Common questions, answered plainly.

What is a monsplasty and what does the surgery involve?

A monsplasty reduces excess skin and fat over the mons pubis, the area above the pubic bone. Through an incision usually placed low so it can sit within underwear, tissue is removed and the area is lifted and flattened. It is sometimes done at the same time as abdominoplasty, or with a labiaplasty. Dr Seyedabadi will explain which approach fits your anatomy and goals.

Would a monsplasty help fullness over my pubic area after pregnancy or weight loss?

It may help. People often notice fullness or overhanging skin in this area after pregnancy, significant weight loss or ageing, which can affect clothing, exercise, hygiene and comfort. Suitability depends on how much of the fullness is skin, fat or both, your general health, whether your weight is stable, and whether you smoke. Dr Seyedabadi will examine you and may suggest a different or staged approach.

What is recovery like after a monsplasty?

Recovery varies, and combining monsplasty with abdominoplasty usually means a longer one. Most people take around two weeks away from work, sometimes more with a physical job. Swelling, bruising, numbness and tightness are common early on and typically improve over weeks to months. Most people avoid heavy lifting and strenuous exercise for about six weeks. You may be asked to wear compression garments and attend several follow up visits.

What are the risks, and how do referrals and fees work?

Risks include bleeding, infection, seroma, delayed wound healing, scarring that may widen, altered sensation, contour irregularity, asymmetry and the possibility of further surgery. Risks specific to your health are discussed at consultation. A GP referral is required before a cosmetic surgery consultation, and a cooling off period applies before your surgery date. Fees are quoted after your consultation, and whether any rebate applies depends on your circumstances.
